I've had some transactions declined at Quidco, as I'm sure happens to everyone sometimes, but apart from that earned £1000s of cashback/bonuses over many years with no significant issues. In my opinion they're fine, just don't try to run multiple accounts, make a Quidco partner into a "daily click" when it's not intended to be used that way, or engage in other such shenanigans.

Update - my second payment should have been made today - its still showing as pending. Anyone else have this?

Whilst I do sympathise, if the first payment was not received, was it perhaps a tad unwise to make a second payment request?

Just a thought but have you treble checked your account details for your request. Maybe you have made an error or even been hacked.

the second payment has now been received and I've used this account lots of times. The first payment is still missing

For reasons unknown, our bank shows the status of this payment as "failed", which means the funds haven't left our account. Apologies for the delay in getting this sorted, the funds have now been added back to your account.
